SYSTEM ADMISSION & SCHOLARSHIP EXAMINATION (MSU-SASE)

APPLICATION FORM

REQUIREMENTS:

1. Certification from High School Principal that the applicant is a Grade 12 student, or Diploma/Grade Card if the student is already a Senior High School graduate, with no college background;
2. Student School ID Card;
3. 2″x2″ ID picture with white background; and
4. Duly accomplished Application Form

INSTRUCTION:

Accomplish this form carefully. Make sure to fill in all required fields and doublecheck that every information is COMPLETE, CORRECT, and ACCURATE. Any wrong or incomplete entry can result in disqualification either in taking the exam or in the admission phase. Application forms with unfilled blanks will be rejected automatically.

OVPAA/MSU-SASE Unit does not make any changes to the submitted information nor your SASE Report of Rating, so review your form well.

REMINDERS:

1. Consult your Parents/Guardians first before selecting a choice in MSU CAMPUS PREFERRED box. Your choice shall be considered Final. Limitation of slots in each Campus restricts applicants from changing the preference in the admission process.
2. All requirements must be uploaded not later than O𝐜𝐭𝐨𝐛𝐞𝐫 1, 2024, 𝐭𝐨 𝐍𝐨𝐯𝐞𝐦𝐛𝐞𝐫 14, 2024. No Application Form will be accepted beyond this deadline.
3. No onsite application will be allowed during the examination day.
4. MSU-SASE is FREE. No Testing Fee shall be collected.
5. Don’t forget to bring your face mask and vaccination card/medical certificate on the day of the exam.
